IMO Shipper's Declaration Form, 4-Part NCR

Our IMO Shippers declaration form helps you meet the requirements set forth in the IMDG Code, chapter 5.4. The shipper must certify that the contents are fully and accurately described by proper shipping name, are classified, packed, marked and labeled, and are in all respects in the proper condition for transport according to the applicable regulations. ICC The Compliance Center provides these forms 8.5" x 11" (21.59 cm x 27.94 cm), in packages of 100. Available in two styles: 4-part snap set NCR blank or preprinted, or blank laser sheets for use with our FormTool® software and maritime FormFiller template.
