Aboriginal Healthy Babies/Healthy Children (HBHC)
 
  • Supports for pregnant women and families with children 0-6 years
  • home visits by nurses and other home visitors help expectant and new parents with having a healthy pregnancy and birth, breastfeeding, nutrition, child development, and being a parent
 
Community Support Services  
 
  • Supports for older adults and adults with disabilities
  • helps people remain independent in their own homes for as long as possible
  • offers a variety of program and services to the general public 
  • referral to other community services
 
Congregate Dining  
Shared mealtimes with hot food and social activities * activities include games, crafts, and physical activities
 
Friendly Visiting and Security Checks  
Regular visits or phone calls to older adults and people with disabilities who live alone, to provide companionship and reassurance
 
Home Help and Homemaking 
Assistance with daily chores, such as laundry, light housekeeping, pet care, errands, shopping, and meal preparation * personal support worker visits weekly
 
Home Maintenance and Repair  
Assistance with snow removal, yard work and minor repairs
 
Medical Transportation  
Coordinates medical transportation services and assists with medical appointments to Parry Sound Mon-Fri and Britt on Wed
 
Community Wellness  
 
Works closely with people and families to reduce the risk and incidence of family violence * culturally appropriate services

  • referrals and advocacy
  • help completing relevant forms and applications
  • supportive accompaniment to consultations
 
Diabetes Healthy Living  
Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices
 
Family Well-being
Ensure the best interests, protection and well being of children and their families
 
Foot Care 
Offers office and in-home foot care services provided by a personal support worker
 
National Native Alcohol and Drug Abuse Program (NNADAP)
Prevention, intervention, and follow up services for people with addictions
 
Respite Care  
 
  • Temporary relief for caregivers caring for people with complex medical conditions
  • May include homemaking, some personal care, light housekeeping, attendant care, and monitoring

Child & Youth Worker

The Child and Youth worker shall promote Magnetawan First Nation’s goals and objectives, maintain the level of professionalism expected by Magnetawan First Nation as outlined in the policies and procedure manuals, and uphold the ethics of the position.
