Community Health Nurse

Community Health Representative
Responsible for planning and assisting in the implementation of healthy activities with the goal to influence community members to alter lifestyle behaviors that contribute to chronic diseases
Crisis Intervention
Provides crisis intervention services to people affected by family violence and suicide such as:
- immediate on site or telephone assistance
- dispatches call to police when deemed necessary
- provides support and assistance to ensure victims safety
- coordinates crisis plan
- offers information sessions and workshops
- provides advocacy
Diabetes Education Program
- Information about living with diabetes
- includes monitoring, blood sugar levels, food choices, exercise, medications, and lifestyle choices
Family Well-being
Ensure the best interests, protection and well being of children
Long Term Care Program
- Supports for older adults, and adults with disabilities
- helps people remain independent in their own homes for as long as possible
- culturally appropriate services for Aboriginals
Lunch and Learn
- Group meals and social activities include a lunch and supper once a month
- activities include guest speakers, information sessions and socializing
- helps to connect people who may be isolated
Medical Transportation Program
- Non-emergency transportation to local and long-distance medical appointments in Timmins, Iroquois Falls, Kirkland Lake, Sudbury and Matheson

- Assists with making arrangements for out-of-town medical appointments, including booking appointments, hotel reservations, and completing applications for mileage compensation
Mental Health Program
- Provides services to people suffering from mental health and addictions through a variety of out-patient and in-patient programs and services
