  • The Health department provides services that promote physical and mental health for community members 
  • Offers primary health care including treatment and emergency services 
  • Coordinates health care services through Nursing staff, Community Health Representative, visiting physicians and Family/Home Support workers 
  • Provides health awareness and counselling, education and prevention workshops especially in the areas of diabetes and alcohol/drug abuse 
  • Coordinates clinics/workshops in areas that may include Diabetes, Immunization, Sexual Health and Communicable Diseases 
  • Offers a palliative care program when required
 
Addresses individual health care needs by providing: 
 
  • Advocacy on behalf of individual with other health professionals  
  • Coordination of one-on-one pre-natal care with the community health nurse  
  • Home visits to the sick, elderly, new mothers and persons with health care needs 
  • Assistance with coordinating local and long distance travel arrangements for medical appointments
 
Coordinates the following programs: 
 
  • Diabetes Program 
  • Early Childhood Development 
  • Elder's Complex 
  • Home Support 
  • Medical-Van Transportation
 
Diabetes Program
 
  • Supports for individuals living with prediabetes or diabetes
  • Provides education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services in cooperation with the Diabetes Team from the St Joseph's Care Group in Thunder Bay 
  • Offers basic foot care services  
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Assists individuals in finding appropriate services  
  • Provides referrals to other health care professionals and the Northern Diabetes Health Network
 
Medical-Van Transportation
 
  • Delivers medical transportation services and assistance in making medical appointment arrangements 
  • Provides transportation to medical appointments within the community  
  • Provides transportation and/or assistance from/to Whitesand First Nation to medical appointments (doctor, dentist, optometrist, specialist) in Sioux Lookout, Thunder Bay and Winnipeg 
  • Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation
 
Home Support Program
 
  • Assists elders and families that have members with disabilities in their residence with routine household activities such as light housekeeping, laundry, shopping, meal preparation and personal care  
  • Provides household support for heavy and seasonal housecleaning  
  • Provides assistance with heavy outdoor work during the winter months such as shovelling and firewood  
  • Acts as respite for caregivers  
  • Provides short-term loan of mobility aids such as walkers and raised toilet seats

BAND MANAGER

The Band Manager will ensure the efficient and effective delivery of all funded programs and services and will provide supervision and leadership for delivery of administrative and program services to the members of Whitesand First Nation.
