- Frail Elderly Person: 60 years old and over (effects of the aging process, a previous illness or injury)
- Disabilities: an individual with physical limitations that will continue for a prolonged period of time. To be determined by a physician.
- Acute illness: an individual with an acute illness that is considered temporary or short term.
- Chronic Illness: an individual with who is frail or disabled as a result of a long-term illness.
- Convalescence: a person who has experienced an acute illness at home or who is discharged from hospital after an accident, illness or injury.
- Referrals will be accepted from the applicant, family members, friends, doctors, nurses, hospital discharge planners and other community resources. The Home Care Nurse will assess the client needs and determine the types of services he or she will require.
- This program addresses the needs of high-risk seniors who require support in meeting their activities of daily living. The staff consist of coordinators, nurses, personal support workers, home makers, and community support services.
- Transportation services Monday to Friday from 1:00 pm to 4:00 pm. Also available during social events and community meetings.

Under the direction, guidance and supervision provided by the Community Health Coordinator, the Community Health Nurse is responsible for planning and delivering Community Health Programs, delivering immunization programs in accordance with the FNIHB Immunization Protocols and Ministry Standards.
Under the general supervision of the Director of Finance, the Collections Officer is responsible for the collection of revenues for all overdue Accounts Receivable debtors of the Moose Cree First Nation.
Under the general supervision of the Director of Health Services, the incumbent shall be responsible for assisting in ensuring that individuals are reporting into the check point.
The Health and Wellness Coordinator is responsible for community mental health programs for the Moose Cree First Nation community.
