ADDICTIONS & WELLNESS
 
Vision
To provide services to individuals and family members in the community of Fort William First
Nation that are affected by addictions and require direction, support, and advocacy in a holistic, culturally sensitive, non-bias manner that is safe and confidential.
 
Mission
To establish the Addiction and Wellness Program in Fort William First Nation community that can address not only the addictions piece, but the overall wellness associated with recovery and healthy living in a culturally relevant manner
To assist and support Fort William First Nation community members in the relapse Prevention, Treatment referral and Aftercare process while maintaining sobriety
To provide a safe and confidential atmosphere for Fort William First Nation community members to initiate services and tailor them to meet the individuals or family members needs

The values implemented in the Addiction and Wellness Program is based on the Seven Grandfather Teachings that were also identified in the Fort William First Nation Healing Strategy Document. They are: Wisdom, Love, Respect, Courage, Honesty, Humility and Truth.

Medical Van Services
 
Services include Transportation for the following appointments within the city of Thunder Bay:
 
  • general practitioners
  • specialists
  • therapy (as referred by a general practitioner or specialist)
  • dental
  • optometry
 
Client Eligibility
 
To be eligible to receive medical transportation benefits under the Non-Insured Health Benefits Program, a person must be:
 
  • A registered Indian according to the Indian Act; or
  • An Inuk recognized by one of the Inuit Land Claim organizations – Nunavut Tunngavik Incorporated, Inuvialuit Regional Corporation, Makivik Corporation or Labrador Inuit Association. For Inuit residing outside of their land claim settlement area, a letter of recognition from one of the Inuit claim organizations and a long form birth certificate are required; or
  • An infant up to one-year-old of an eligible parent;
  • Must reside on Fort William First Nation

 

Operating Policies/ Procedures- Non Emergency Medical Van

  • Patients requesting services must arrange their own appointments according to the following restrictions:  Monday to Friday between the hours of 9am and 4 pm.
  • Patient shall inform the health center of their appointment the24 hours before the appointment.
  • The following information is required at the time of booking the medical van (date of the appointment, time of the appointment, name of the  doctor being seen, Clients Status number, phone number and address)
  • Personal use of the medical van is prohibited.
  • Patients shall be picked up at the location specified at the time of medical van booking.  The driver shall wait no more than five minutes past the pickup time agreed to, if no one seems to be coming, the driver shall leave and continue on his regular scheduled route.  The patient is then responsible for their own transportation for that appointment.
  • Clients are responsible to stay at the facility of their appointment when they HAVE finished their appointment. If the client leaves the facility they will not be eligible for transportation back to the First Nation.
  • At no time shall alcohol, intoxicants, and or non-prescription drugs be transported in the medical van.
  • The driver maintains the right to refuse service to any person found to be in possession of or under the influence of alcohol, intoxicants, and or non-prescription drugs.
  • The van driver is not responsible for looking after children while parents/guardians are seeing the doctor.  Patients are responsible for making suitable child care arrangements.
  • Baby/child safety restraints shall be made available.  All passengers must wear seat belts and or restraints at all times while the van is in operation.
  • Smoking is not permitted on the medical van at anytime.
  • No food or beverages are permitted on the medical van at any time.
  • There will be no unscheduled stops
  • Anyone under the age of sixteen(16) must be accompanied by an adult (18 years of age and older)
  • In a true emergency situation, an ambulance should be called.
  • Complaints should be made in writing and signed by the patient. Complaints should be submitted to the Health Center Manager.
  • This transportation policy shall be posted in the Health Center and available in the van, on request.
  • All information pertaining to patients shall be held in the strictest confidence.
 
Taxi Slip (After hours Services)
 
Services include transportation to Thunder Bay Regional Health Sciences Center with a stop at the pharmacy if required.
 
Client Eligibility
 
To be eligible to receive medical transportation benefits under the Non-Insured Health Benefits Program, a person must be:
 
  • A registered Indian according to the Indian Act; or
  • An Inuk recognized by one of the Inuit Land Claim organizations – Nunavut Tunngavik Incorporated, Inuvialuit Regional Corporation, Makivik Corporation or Labrador Inuit Association. For Inuit residing outside of their land claim settlement area, a letter of recognition from one of the Inuit claim organizations and a long form birth certificate are required; or
  • An infant up to one-year-old of an eligible parent;
  • Must reside on Fort William First Nation (medical van, taxi slip after hours).With the exception of members of Fort William First Nation that are 65 years +.
Operating Policies/ Procedures- Taxi Slips/ Afterhours
 
  • For medical transportation after  4:00 pm and before 9:00 am weekdays and 24 hours Saturday and Sunday, clients shall telephone the On-call worker at 472.7701 for medical transportation that does not require an ambulance.
  • True Emergency situations- an ambulance should be called
  • Patients receiving taxi slips must provide a verification slip signed by the attending physician at the emergency the day following the visit to the emergency.  Failure to provide the signed verification slip will result in the suspension of services.
  • Clients who do not hand in their verification slips will be charged the dollar amount of the taxi service and suspension of services will occur until the payment has been received by Fort William First Nation.
  • Clients who do not use a taxi that has been called on their behalf will be charged a fee of $10.00 and a suspension of taxi services will occur until payment is received by Fort William First Nation.
 
Medical Transportation out of Thunder Bay Area
 
Medical Transportation Benefits may be provided for clients to access the following types of medically required health services.
 
  • Medical services defined as insured services by provincial/territorial health plans (eg. Appointments with physician, hospital care)
  • Diagnostic tests and medical treatments covered by provincial/territorial health plans
  • Alcohol, solvent, drug abuse and detox treatment
  • Traditional healers and
  • Non-Insured Health Benefits (vision, dental, mental health)
 
Client Eligibility

Operating Policies/ Procedures- Medical Transportation out of Thunder Bay
 
Contact the Community Health Center or First Nation and Inuit Health Branch local office at 807-345-1443 (phone) 807-343-9196 (fax).  This must be done two weeks prior to appointment date.
 
The following information will be required to process the application through NIHB.
 
  • Documentation with appointment information (date, time, location, name of health care professional being seen)
  • Client information (name, date of birth, status number, address with postal code)
  • Referral information (name of referring health care professional, specialty)
  • Documentation from referring health care professional or health care professional being seen stating that an escort is medically required.
  • Name of the escort if an escort is required.
 
The information provided is faxed to NIHB local office that processes it for approval.  Approval or denial of an application is forwarded to Fort William First Nation.  Client is informed of decision by Health Centre Staff.
 
Medical transportation benefits include ground, water and air travel, meals and accommodations.
 
FAMILY CENTRE – EARLY YEARS PROGRAM
 
The Family Centre would like to take this opportunity to inform families that we are in hopes of doing more virtual learning. As programming is at a stand still due to COVID-19, we want to provide our children and families with a variety of resources that you can do from home. We would love your input in providing us with any ideas that are of interest to you and your children. Your feedback would be greatly appreciated.

Project Coordinator (6 Month Contract)

Under the supervision of the Health Manager, the Project Coordinator will work along with the Cultural and Recreation Coordinator on a part-time basis with a wide range of duties for the design of a Red Jingle Dress to be displayed at our Annual First Nation pow-wow honouring our Missing and Murdered Women, Girls and LGBTQ2S families.

Accounts Receivable Clerk

The Accounts Receivable (A/R) Clerk is directly responsible for performing data entry and related duties in posting accounts receivable payments.
