Health Services
- Provides services that promote physical and mental health for community members
- Coordinates consultations with visiting professionals including a Nurse Practitioner and a Community Health Nurse
- Provides health awareness, education and prevention workshops and seminars
- Coordinates clinics in areas that may include Diabetes, Immunization, Sexual Health and Communicable Diseases
- Coordinates regular foot care sessions with visiting chiropodist
- Identifies and attempts to correct health hazards
Addresses individual health care needs by providing:
- Intake services to determine health care needs of individual
- Referrals to appropriate health care providers based on information in the intake assessment
- Advocacy on behalf of individual with other health professionals
- Coordination of one-on-one pre-natal care with the community health nurse
- Home visits to the sick, elderly, new mothers and persons with health care needs
- Assistance with coordinating local and long distance travel arrangements for medical appointments
Aboriginal Healthy Babies/Healthy Children
- Delivers a parenting support program and promotes healthy child development
- Provides group information and educational sessions on topics about healthy pregnancy, birth and taking care of baby at home
- Provides information on the stages of pregnancy
- Offers parents handbook of pregnancy and baby care (Baby's Best Chance)
Program participants are provided with:
- Follow up via telephone within 48 hours of discharge from hospital
- Home visits pre birth to prepare family for new arrival
- Home visits post birth to offer support to new mothers
- Milk coupon program
- Home visits during child's early years to offer development guidance
- Referrals to other programs and services
Canada Prenatal Nutrition Program
Provides supports and services to help families improve maternal and infant nutritional health.
Focuses on the following 3 areas:
Maternal Nourishment that may include:
- Baby food making and nutritional cooking sessions
- Healthy snacks and meal information
- Milk and food vouchers as well as vitamin supplements
Nutrition Screening, Education and Counselling includes:
- Education on prenatal nutrition
- Understanding food labels
- Presentations by dietitians and health specialists
- Nutrition screening by Community Health Representative
- Individual counselling by nutritionists
Breastfeeding Program includes:
- Promotion of breastfeeding initiation and duration
- Support groups
- Breastfeeding videos and resource materials
Aboriginal Diabetes Initiative
- Provides education and services for the prevention, management and treatment of diabetes
- provides case management and screening services
- works to increase early detection and to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- advises on developing and implementing an approach to care and management of diabetes
- promotes increased physical activity and knowledge of healthy eating
- provides workshops, presentations and hands-on training for cultural activities and traditions
- assists individuals in finding appropriate services
- provides referrals to other health care professionals and the Northern Diabetes Health Network (NDHN)
Community Wellness Program
- Supports reduction or prevention of family and community violence
- Provides victim service referrals and emergency support for family violence
- Works one-on-one with families experiencing family violence to develop a plan of action for identified concerns
- Provides referrals to counselling, legal services, and treatment centres
- Offers Aboriginal culture-based healing, illness prevention, and crisis intervention workshops
- Organizes and facilitates community events in schools, powwows, and health fairs to promote a healthy lifestyle and prevention of family violence
- Develops programs, workshops, promotions, and prevention education campaigns dealing with family-based violence awareness
Home and Community Care Program
- Provides health care services for ill, injured or elderly community members in their own homes
- Provides the following services through a Case Manager/Nurse, Home Support Workers and Homemakers:
- assessment to determine individuals health history and develop a personalized care plan
- case management to ensure that care plans are being carried out and that the plan developed is still suitable
- home care nursing duties
- access to medical supplies and equipment
- personal care help with activities such as dressing and bathing
- basic or advanced practice foot care
- teaching the individual in need to care for themselves
- home support with light housekeeping, laundry and meal preparation
- respite for family members and caregivers
- help with light outdoor/yardwork
Medical Transportation Program
- Delivers medical transportation services and assistance in making medical appointment arrangements
- provides transportation to medical appointments within the community and surrounding area
- provides transportation and/or assistance from/to Wabauskang First Nation to medical appointments (doctor, dentist, optometrist, specialist) in Red Lake, Ear Falls, Kenora and Dryden
- provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation
National Native Alcohol and Drug Abuse Program
- Provides services and support for individuals and families struggling with addiction issues
- Offers Alcoholics Anonymous (AA) and Narcotics Anonymous (NA) meetings when required
- Provides transportation to all meetings and workshops upon request
Prevention
- Provides public seminars/meetings to raise community awareness of alcohol, drug abuse and gambling addictions
- Holds educational sessions in schools
- Coordinates cultural and spiritual events
Intervention
- Facilitates recreational activities and social events
- Provides individual and group discussions
- Offers cultural and spiritual gatherings
Aftercare Services
- Provides the following services aimed at preventing alcohol and drug abuse problems from reoccurring:
- Counselling, sharing circles and support groups
- Regular phone calls and home visits
- Referrals to Band services, social services, detoxification and treatment centres and medical assistance
