  • Provides services that promote physical and mental health for community members
  • Coordinates consultations with visiting professionals including a Nurse Practitioner and a Community Health Nurse
  • Provides health awareness, education and prevention workshops and seminars
  • Coordinates clinics in areas that may include Diabetes, Immunization, Sexual Health and Communicable Diseases
  • Coordinates regular foot care sessions with visiting chiropodist
  • Identifies and attempts to correct health hazards
 
Addresses individual health care needs by providing:
 
  • Intake services to determine health care needs of individual
  • Referrals to appropriate health care providers based on information in the intake assessment
  • Advocacy on behalf of individual with other health professionals
  • Coordination of one-on-one pre-natal care with the community health nurse
  • Home visits to the sick, elderly, new mothers and persons with health care needs
  • Assistance with coordinating local and long distance travel arrangements for medical appointments
 
Aboriginal Healthy Babies/Healthy Children
 
  • Delivers a parenting support program and promotes healthy child development
  • Provides group information and educational sessions on topics about healthy pregnancy, birth and taking care of baby at home
  • Provides information on the stages of pregnancy
  • Offers parents handbook of pregnancy and baby care (Baby's Best Chance)
 
Program participants are provided with:
 
  • Follow up via telephone within 48 hours of discharge from hospital
  • Home visits pre birth to prepare family for new arrival
  • Home visits post birth to offer support to new mothers
  • Milk coupon program
  • Home visits during child's early years to offer development guidance
  • Referrals to other programs and services
 
Canada Prenatal Nutrition Program  
 
Provides supports and services to help families improve maternal and infant nutritional health.
Focuses on the following 3 areas:
 
Maternal Nourishment that may include:
 
  • Baby food making and nutritional cooking sessions
  • Healthy snacks and meal information
  • Milk and food vouchers as well as vitamin supplements
 
Nutrition Screening, Education and Counselling includes:
 
  • Education on prenatal nutrition
  • Understanding food labels
  • Presentations by dietitians and health specialists
  • Nutrition screening by Community Health Representative
  • Individual counselling by nutritionists
 
Breastfeeding Program includes:
 
  • Promotion of breastfeeding initiation and duration
  • Support groups
  • Breastfeeding videos and resource materials
 
Aboriginal Diabetes Initiative 
 
  • Provides education and services for the prevention, management and treatment of diabetes
  • provides case management and screening services
  • works to increase early detection and to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • advises on developing and implementing an approach to care and management of diabetes
  • promotes increased physical activity and knowledge of healthy eating
  • provides workshops, presentations and hands-on training for cultural activities and traditions
  • assists individuals in finding appropriate services
  • provides referrals to other health care professionals and the Northern Diabetes Health Network (NDHN)
 
Community Wellness Program 
 
  • Supports reduction or prevention of family and community violence
  • Provides victim service referrals and emergency support for family violence
  • Works one-on-one with families experiencing family violence to develop a plan of action for identified concerns
  • Provides referrals to counselling, legal services, and treatment centres
  • Offers Aboriginal culture-based healing, illness prevention, and crisis intervention workshops
  • Organizes and facilitates community events in schools, powwows, and health fairs to promote a healthy lifestyle and prevention of family violence
  • Develops programs, workshops, promotions, and prevention education campaigns dealing with family-based violence awareness
 
Home and Community Care Program 
 
  • Provides health care services for ill, injured or elderly community members in their own homes
  • Provides the following services through a Case Manager/Nurse, Home Support Workers and Homemakers:
  • assessment to determine individuals health history and develop a personalized care plan
  • case management to ensure that care plans are being carried out and that the plan developed is still suitable
  • home care nursing duties
  • access to medical supplies and equipment
  • personal care help with activities such as dressing and bathing
  • basic or advanced practice foot care
  • teaching the individual in need to care for themselves
  • home support with light housekeeping, laundry and meal preparation
  • respite for family members and caregivers
  • help with light outdoor/yardwork
 
Medical Transportation Program  
 
  • Delivers medical transportation services and assistance in making medical appointment arrangements
  • provides transportation to medical appointments within the community and surrounding area
  • provides transportation and/or assistance from/to Wabauskang First Nation to medical appointments (doctor, dentist, optometrist, specialist) in Red Lake, Ear Falls, Kenora and Dryden
  • provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation
 
National Native Alcohol and Drug Abuse Program  
 
  • Provides services and support for individuals and families struggling with addiction issues
  • Offers Alcoholics Anonymous (AA) and Narcotics Anonymous (NA) meetings when required
  • Provides transportation to all meetings and workshops upon request
 
Prevention
 
  • Provides public seminars/meetings to raise community awareness of alcohol, drug abuse and gambling addictions
  • Holds educational sessions in schools
  • Coordinates cultural and spiritual events
 
Intervention
 
  • Facilitates recreational activities and social events
  • Provides individual and group discussions
  • Offers cultural and spiritual gatherings
 
Aftercare Services
 
  • Provides the following services aimed at preventing alcohol and drug abuse problems from reoccurring:
  • Counselling, sharing circles and support groups
  • Regular phone calls and home visits
  • Referrals to Band services, social services, detoxification and treatment centres and medical assistance
