Mission Statement
 
The Jane Mattinas Health Centre provides health and prevention services that focuses on educational aspects of healthy living in a manner that promotes mutual respect in a safe environment.

To have a community that practices daily activities. Maintains a constant awareness of the positive benefits of proactive health care principles for the mind, body & soul.

Health centre focused on family and community health:
 
  • Offers primary health care services through a medical clinic and outreach programs
  • Provides services through Physicians, Nurse Practitioners, RNs, RPNs and traditional healers
  • Offers culturally appropriate programs and services for Aboriginals, combining traditional healing and Western medical practices
  • Provides chronic disease management
  • Provides referrals to other service providers and specialists
  • Performs some diagnostic procedures including electrocardiograms and specimen collection
  • Publishes a monthly newsletter available online
  • Collaborates with Riverside Health Care Facilities and Fort Frances Family Health Team for consultations, diagnostic imaging and laboratory analysis

Cultural Support Band Representative

Under the Direct Supervision of the Band Representative, the Cultural Support Worker is responsible for promoting and ensuring that families receiving child welfare services are delivered in a manner which is both respectful of and congruent with the culture of Constance Lake First Nation. 

Community Support Band Representative

Under the direct supervision of the Community Band Representative, Constance Lake First Nation requires a Community Support Band Representative for Child Welfare to advocate/represent registered/affiliated with First Nation members that are involved with any/all Child Welfare Agencies throughout Canada. 

Band Representative Office Assistant

Under the direct Supervision of the Community Band Representative, the Band Representative Assistant will provide assistance to the Band Representative Program in prevention, intervention awareness and administrative services to the community members of Constance Lake First Nation.
