Medical Centre
- Provides services that promote physical and mental health for community members
- Provides consultations with visiting physicians
- Provides health awareness, education and prevention workshops and seminars
- Coordinates clinics in areas that may include Diabetes, Immunization, Sexual Health and Communicable Diseases
- Coordinates monthly foot care sessions in individual's home or at office
- Works with maintenance worker to ensure community water quality
- Identifies and attempts to correct health hazards
Addresses individual health care needs by providing:
- Intake services to determine health care needs of individual
- Referrals to appropriate health care providers based on information in the intake assessment
- Advocacy on behalf of individual with other health professionals
- Coordination of one-on-one pre-natal care with the community health nurse
- Home visits to the sick, elderly, new mothers and persons with health care needs
- Assistance with coordinating local and long distance travel arrangements for medical appointments
Diabetes Education Program
Information about living with diabetes: includes monitoring, blood sugar levels, food choices, exercise, medications, and lifestyle choices.
Home and Community Care
Health services provided in a home or community setting for older adults, persons with disabilities, or injured persons: includes nursing care, personal support, foot care, homemaking, meal preparation, and respite care.
Medical Transportation Program
- Delivers medical transportation services and assistance in making medical appointment arrangements
- Provides transportation to medical appointments within the community
- Provides transportation from/to Angle Inlet and Dog Paw communities to any medical appointments (doctor, dentist, optometrist etc) in the city of Kenora or Winnipeg
- Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation
Mental Health and Addictions
Offers a variety of mental health and addictions services for community members
Programs and services can include the following:
Addictions and Gambling Program
- Offers community treatment and education
- Provides assessment, referral and aftercare
- Offers individual relapse prevention counselling
- Gives individual, couples and family treatment therapy
- Provides support for family members affected by addictions
Case Management Program
- Provides individual counselling for adults struggling with physical, medical and emotional health issues
- Offers assistance through support workers in life skills including housing, self care, nutrition, shopping, money management, social and recreational living
- Provides family support and education
Mental Health Program
- Offers individual, couple, family and group counselling for stress management and crisis intervention
- Provides referral services to appropriate supports
