Central Booking & Collection Sites

To book drug and alcohol tests, or medicals, you may call central booking anytime Monday to Friday 8:00am to 8pm ET. Bilingual service is available at this number.

Appointments at any of DriverCheck's collection sites nationwide can be booked via one central number. Central booking will recommend the nearest or most convenient site. Most sites require an appointment for ease in confirming company, donor and test type.

 

Donors need to bring a piece of photo ID. It is also important to note that for drug tests, donors must be ready to provide a urine sample in order to avoid waiting. Donors unable to provide a sample at the time of appointment or who provide an insufficient sample can expect to stay up to three hours and drink up to 40 ounces of fluids to provide an adequate sample.

 

Our collection sites will provide the donor with a Drug Testing Custody & Control Form (CCF). Its 5 parts are distributed as follows:

Copy 1 - goes to the lab with the specimen (no donor signature is recorded on this copy in order to ensure anonymity when testing at the lab)
Copy 2 - has the donor’s signature and is faxed and mailed to the MRO
Copy 3 - is retained by the collection site
Copy 4 - is mailed to you, the employer
Copy 5 - is retained by the donor

 

Urine samples will be shipped by coast to coast courier.

For next day results, collection must take place before the courier arrives for that day's samples. Negative tests without complication collected from our high speed sites across Canada will be reported to your company before 5pm the next day if your company has a confidential method of receiving results. The collection site will fax a copy of the chain of custody form to DriverCheck's MRO for entry into our system before the result arrives in order to expedite the verification process. Results will be processed on receipt from the lab.

 

Alcohol tests are done using either a saliva or breath sample with DOT approved testing devices. Results are immediate.
