  • Provides services that promote physical and mental health for community members
  • Coordinates health care services through nursing station
  • Provides physician consultations a few days per month with a visiting physician from Sioux Lookout
  • Provides health awareness and counselling, education and prevention workshops especially in the areas of diabetes and alcohol/drug abuse
  • Coordinates clinics/workshops in areas that may include Diabetes, Immunization, Sexual Health and Communicable Diseases
  • Offers a palliative care program when required
 
Addresses individual health care needs by providing:
 
  • Advocacy on behalf of individual with other health professionals
  • Coordination of one-on-one pre-natal care with the community health nurse
  • Home visits to the sick, elderly, new mothers and persons with health care needs
  • Assistance with coordinating local and long distance travel arrangements for medical appointments
 
Coordinates the following programs through the Nursing Station:
 
  • Diabetes Program
  • Immunization Clinic
  • Maternal Child Health Program
  • Mental Health Program
  • National Native Alcohol and Drug Abuse Program
  • Travel Clerk
 
Ontario Telemedicine Network (OTN)
 
  • Provides access to specialized medical care using videoconferencing and other tele-diagnostic equipment
  • Allows specialists to remotely examine and prescribe treatments to patients in their home communities
  • Reduces the need to travel to receive care
 
Home and Community Care Building - Home and Community Care Program  
 
Provides health care services for ill, injured or elderly community members in their own home. Provides the following services through a Case Manager/Nurse, Personal Support Workers and Homemakers:
 
  • Assessment to determine individuals health history and develop a personalized care plan
  • Case management to ensure that care plans are being carried out and that the plan developed is still suitable
  • Home care nursing duties
  • Personal care help with activities such as dressing and bathing
  • Basic or advanced practice foot care
  • Teaching the individual in need to care for themselves
  • Home support with light housekeeping, laundry and meal preparation
  • Respite for family members and caregivers
  • Help with light outdoor/yardwork
 
Diabetes Program 
 
Provides education and services for the prevention, management and treatment of diabetes.
 
  • Provides case management and screening services
  • Offers basic foot care
  • Offers walking programs and exercise programs tailored to meet the needs of the individual
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals and the Northern Diabetes Health Network
 
Immunization Clinic 
 
Provides inoculations or other measures to prevent people from contracting specific controllable diseases.
 
Maternal Child Health Program  
 
Provides services and programs to enhance the lives of pregnant women and families with infants and young children.
 
Offers education and information on various topics including:
 
  • Fetal Alcohol Spectrum Disorder (FASD) awareness
  • Child nutrition issues
  • Healthy cooking and shopping for families
  • Prenatal nutrition
 
National Native Alcohol and Drug Abuse Program
 
Provides services and support for individuals and families struggling with addiction issues.
 
  • Offers Alcoholics Anonymous (AA) and Narcotics Anonymous (NA) meetings when required
  • Provides transportation to all meetings and workshops upon request
 
Prevention
  • Provides public seminars/meetings to raise community awareness of alcohol, drug abuse and gambling addictions
  • Holds educational sessions in schools
  • Coordinates cultural and spiritual events
 
Intervention
  • Facilitates recreational activities and social events
  • Provides individual and group discussions
  • Offers cultural and spiritual gatherings
 
Aftercare provided for preventing reoccurrence:
  • Counselling, sharing circles and support groups
  • Regular phone calls and home visits
  • Referrals to band services, social services, detoxification and treatment centres and medical assistance
 
Travel Clerk  
 
Responsible for processing Non-Insured Health Service claims for community and Band members.
 
  • Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
  • Assists with making travel arrangements
  • Maintains client documentation, receipts and related information
