  • Nursing station providing basic medical and health care services that promote physical and mental health for community members
  • Coordinates health care services through the Nursing Station; health team consists of Community Health Nurses and Representatives, a visiting Physician and Crisis Team workers
  • Provides health awareness and counselling, education and prevention workshops/clinics in areas that may include Child Care, Diabetes, Elderly & Chronic Disease, Immunization, Sexual Health and Communicable Diseases
  • Coordinates services of a visiting dentist
  • Addresses individual health care needs by providing:
  • Treatment of on-going health problems
  • Emergency services
  • Advocacy on behalf of individual with other health professionals
  • Coordination of one-on-one pre-natal care with the community health nurse
  • Home visits to the sick, elderly, new mothers and persons with health care needs
  • Assistance with coordinating local and long distance travel arrangements for medical appointment
 
Ontario Telemedicine Network (OTN)
 
  • Provides access to specialized medical care using videoconferencing and other tele-diagnostic equipment
  • Allows specialists to remotely examine and prescribe treatments to patients in their home communities
  • Allows consultations for patients with specialty services such as physio, diabetes, etc.
  • Provides opportunity for workshops and education sessions for staff and patients
  • Reduces the need to travel to receive care
 
Aboriginal Healthy Babies/Healthy Children  
 
  • Delivers a parenting support program and promotes healthy child development
  • Provides group information and educational sessions on topics about healthy pregnancy, birth and taking care of baby at home
  • Provides information on the stages of pregnancy
  • Offers parents handbook of pregnancy and baby care (Baby's Best Chance)
 
Program participants are provided with:
 
  • Follow up via telephone within 48 hours of discharge from hospital
  • Home visits pre birth to prepare family for new arrival
  • Home visits post birth to offer support to new mothers
  • Home visits during child's early years to offer development guidance
  • Referrals to other programs and services
 
Diabetes Program
 
  • Provides education and services for the prevention, management and treatment of diabetes
  • provides case management and screening services
  • basic foot care clinics
  • nutrition programs and recipe exchanges
  • works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • advises on developing and implementing an approach to care and management of diabetes
  • assists people in finding appropriate services
  • provides referrals to other health care professionals and the Northern Diabetes Health Network
 
Fetal Alcohol Spectrum Disorder Program  
 
  • Provides education and information on the consequences of alcohol consumption during pregnancy
  • Offers individual and group counselling sessions
  • Offers parenting classes
  • Provides information sessions for teens
 
Advises on the following:
 
  • Alcohol Related Birth Defects (ARBD)
  • Alcohol Related Neurodevelopmental Disorder (ARND)
  • Fetal Alcohol Effects (FAE)
  • Fetal Alcohol Syndrome (FAS)
  • Partial Fetal Alcohol Syndrome (PFAS)
 
Home and Community Care - Physiotherapy Services - Progression and Maintenance  
 
Physiotherapy services may be offered in-home or at the Nursing Station, depending on client. In-home rehabilitative care to help people preserve or regain independence. Physiotherapy services may be offered in home or at nursing station depending on client
 
Services may include:
 
  • physiotherapy
  • occupational therapy
  • speech-language pathology
  • case management
 
National Native Alcohol and Drug Abuse Program
 
  • Provides services and support for individuals and families struggling with addiction issues
  • Offers Alcoholics Anonymous (AA) and Narcotics Anonymous (NA) meetings when required
  • Provides transportation to all meetings and workshops upon request
 
Prevention
 
  • Provides public seminars/meetings to raise community awareness of alcohol, drug abuse and gambling addictions
  • Holds educational sessions in schools
  • Coordinates cultural and spiritual events
 
Intervention
 
  • Facilitates recreational activities and social events
  • Provides individual and group discussions
  • Offers cultural and spiritual gatherings
 
Aftercare Services
 
  • Provides the following services aimed at preventing alcohol and drug abuse problems from reoccurring:
  • Counselling, sharing circles and support groups
  • Regular phone calls and home visits
  • Referrals to band services, social services, detoxification and treatment centres and medical assistance
