Healthy Babies/Healthy Children Coordinator

Under the immediate supervision of the Nurse Manager, Community Health Program and technical direction of a designated CHN, fulfill the mandate of the HBHC Program; provide program services, cultural education, health education, emotional support, and home visits to “at-risk” and “high-risk” families involved in CHP services. Participate as a full functioning Community Health Program (CHP) team member in accordance with the Health mission statement, relevant CHP goals and objectives as well as WIFN philosophies, policies, and regulations.
Requirements:
• Minimum of 5 years working within the WIFN community.
• Minimum of 5 years life-experience with positive child rearing practices.
• Experience with alternative and native traditional healing and wellness approaches.
• Experience facilitating meetings and group activities.
• Experience working in a team environment in a related health care setting (e.g., community).
• Traditional Aboriginal Healing Methods Diploma, Social Service Worker Diploma, or other relevant diploma.
• Group facilitation knowledge and skills.
• Food Educator certification an asset
• Must live and promote a healthy lifestyle, free of alcohol and substance use, and be a positive family and community role model.
• Ability to work flexible hours.
• Possess a valid Ontario Drivers License and reliable vehicle.
• Successful candidate will provide a clear Criminal Vulnerable Sector check on a regular basis
• Program mandate requires a person of native ancestry.
Responsibilities:
• To make home visits to “at risk” and “high risk” families with children 0-6 years to assist the CHN with cultural education, health education, emotional / mental support, advocacy and referrals as required.
• To adhere to documentation, accountability, confidentiality, and professional conduct standards of the HBHC Program.
• To maintain a current, accurate knowledge of the Lay Home Visitor Education Curriculum and the roles and responsibilities of the Lay Home Visitor.
• To actively participate in on-going education activities offered by the HBHC Program.
• To complete community awareness and public relation activities on the HBHC Program within the community and local surrounding agencies as required.
• To adhere to the WIFN philosophy, personnel policy, and CHP mandate.
• To effectively complete delegated tasks and responsibilities, keeping the CHN apprised of client cases, completed follow-up and client outcomes.
• To be able to work flexible hours in order to assist with meeting program client needs.
• To complete and submit a staff month-end program activity and client contact report to the Nurse Manager on the last Friday of each month.
• To upgrade knowledge and skills relevant to the duties and responsibilities associated with the job description such as HBHC Lay Home Visitor and the Partners In Parenting Education Program as required.
• To coordinate family-focused native traditional healing and wellness activities based on program client needs.
