Transitional Aged Youth Worker (Union)

• Diploma in Child and Youth Work or Bachelor of Social Work with previous experience with First Nations communities;
• Experience in working within culturally appropriate service models for First Nations people;
• Demonstrate excellent verbal and written communication skills in order to effectively liaise with clients and a variety of internal and external professionals, the community, agencies and organizations;
• Able to provide on-call services when required;
• Must be willing to work flexible hours;
• Must be willing to travel and cover the entire District, and have access to a reliable vehicle with a $2 million rider liability; and
• Must possess a valid “G” Ontario driver’s license.

• Working with youth in care and that are in the Continued Care and Support for Youth (CCSY) Program and supporting youth in their successful transition out of the child welfare system into adulthood;
• Develop the Continued Care and Support for Youth agreement with the youth and assist the youth in setting goals in the youth plan;
• Monitor the youth’s progress in meeting the goals that were set in the youth plan;
• Collaborating with Children’s Services Workers in supporting youth that are in case and residing in room and board arrangements and independent living arrangements;
• Commitment to providing services in ways that respect First Nations cultural and spiritual practices;
• Knowledge of case management systems, reporting procedures and responsibilities and the ability to work within those systems;
• Effective written and oral communication skills, problem solving, advocacy and assessment skills;
• Ability to manage crisis and adverse situations with staff, youth and families;
• Ability to work within a multi-disciplinary team, as well as manage and maintain public inquiries;
• Knowledge and understanding of health and safety practices related to outreach work and Universal Care precautions;
• Knowledge and understanding of the Child and Family Services Act, the Mental Health Act and other relevant legislation; and
• Ability to work within the policies, standards, procedures and the vision, mission and core values of Kunuwanimano Child and Family Services.
